LOG 34 PA ID NO. 097-UUM2F- 00 DATE 10-05-2016 CATEGORY E FEMA 4116 – DR -IL APPLICANT: FOX WATERWAY AGENCY WORK COMPLETE AS OF: 06-28-2016 : 0 % Site 1 of 1 DAMAGED FACILITY: Administration / Service Building COUNTY: Lake LOCATION: PA-05-IL-4116-PW-00611(0): The address of this damaged facility is 45 South Pistakee Lake Road in Fox Lake, Illinois. Current Version: LATITUDE: 42.39394 LONGITUDE: -88.18601 DAMAGE DESCRIPTION AND DIMENSIONS: PA-05-IL-4116-PW-00611(0): During the incident period starting April 16, 2013 and ending May 5, 2013, severe weather including heavy rainstorms and strong gusty winds passed over the Fox Waterway Agency which is located in Fox Lake, Illinois. The heavy rainstorms resulted in the Fox River overflowing its banks creating fast flowing overland floodwaters. The floodwaters entered into the Fox Waterway Agency Administration/Service Building located at 45 S. Pistakee Lake Road in Fox Lake, IL. This building is a Quonset hut type metal building which houses the Fox Waterway Agency administration and service departments. The facility measures (60 ft X 160 ft) on the exterior. The overland floodwaters made entry through various access doors and garage doors. The floodwaters are estimated to have reached (2 inches to 1 foot) depending on which end of the building is identified. The floodwaters damaged the following: (2,572 SF) of (3/4 inch) plywood subfloor; (612 LF) of base molding; (2,420 SF) of (6 inch thick) wall insulation; (12) interior solid core doors with frames; (2,420 SF) of gypsum wall board; (3) bathroom cabinet vanities; (6 LF) of base kitchen cabinets; (6 LF) of cabinet countertop with backsplash; (12 LF) of custom office base cabinets; (272 SF) of linoleum flooring and (360 SY) of commercial grade carpet. All damaged infrastructure items were either warped and/or soiled beyond repairs. In addition, (1.23 M.B.F.)(Metric Board Foot) of (2 in X 10 in) wood floor joists were destroyed during the emergency response. These joists were cut out and disposed of to allow for dewatering and mold inspections. The cost for the removal of these joists are included in Sub-Grant Application (UUM2F02 – Emergency Protective Measures).

WORK TO BE COMPLETED The Sub-Grantee plans to utilize contract services to perform the following work: mobilize; rent (2) (40 CY) dumpsters for demolition; perform (5,000a SF) of mold remediation; clean and power wash (2,572 SF) of floor space; import and install 1.23 M.B.F.(Metric Board Foot) of (2 in X 10 in) wood floor joists; import and install (2,572 SF) of plywood subfloor; import and install (612 LF) of base molding; import and install (2,420 SF) of (6 in) wall insulation; import and install (3) bathroom cabinet vanities; import and install (6 LF) of base kitchen cabinets; import and install (6 LF) of cabinet countertop with backsplash; import and install (12 LF) of custom office base cabinets; import Federal Emergency Management Agency E-Grants Page 1 of 4 https://sso.fema.net/emmie/internalIntegration?applicationId=345007 10/17/2016 and install (272 SF) of linoleum flooring; import and install (360 SY) of commercial grade carpet; paint (612 LF) of base molding; paint (2,420 SF) of walls and demobilize.

PROCUREMENT: 44 CRF §13.36 Procurement; (b) Procurement standards; (1) Grantees and Sub-Grantees will use their own procurement procedures which reflect applicable State and local laws and regulations, provided that the procurements conform to applicable Federal law and the standards identified in section 13.36. The Sub-Grantee is required to adhere to State Governmental Procurement rules and regulations and maintain adequate records to support the basis for all purchasing of goods, materials and contracting services for projects approved under the Public Assistance Program. Units of work listed in the bid solicitation should match the units identified in the approved scope of work. If less than three (3) bids are obtained, State and FEMA approval is recommended prior to proceeding.

LARGE PROJECTS: This is a large project and final funding will be based on actual costs incurred by the Sub-Grantee for completing eligible work specifically approved in the scope of work for this project. 44 CFR 206.203.1 states “Federal funding shall equal the Federal share of actual costs documented by the grantee.”
